Outpatient Chemical Dependency Program

About Our Program

Our CD program offers 24-hour on call support, and is designed to provide quality treatment and education to adolescents with chemical dependency issues.

Eligibility

Male and female adolescents, 10 to 19 years of age, are eligible with completion of the following:

Chemical Health Assessment
Diagnosis of "Chemically Abusive or Dependent"

Program Phases

Phase 1 (4 weeks)

Intake and orientation
Visual life story
Education and group process

Phase 3 (4 weeks)

Completion of the 5th Step of Recovery
Development of an aftercare plan
Utilizing support networks
Medallion ceremony

Phase 2 (4 weeks)

Identify and apply the Step Works of Recovery
Individual treatment planning
Introduction of support networking

Phase 4 (6 weeks)

Maintain group support
Complete daily journal entries and share with group peers on a weekly basis
Participants remain in contact with peers and support staff as needed

Education Class

This class emphasizes the effects of chemical use on the mind and body. During the course, the participants receive an introduction to the "world of recovery", view videos, participate in group discussions and writing assignments.
