in-home family services

Every family and child has a different story. But sometimes circumstances or choices can diminish the impact of a person's story. At North Homes we are in business to help children build a stronger story. That is why we:

  • provide comprehensive care and prevention services for at-risk youth and their families
  • tailor our programs to match each child's needs and required outcomes
  • meet kids where they're at
  • work to successfully maintain children with their families and community
  • or to successfully return children to their families, communities and independent living

Children's Therapeutic Services and Supports (CTSS)
CTSS works to strengthen families to reduce out-of-home placements. Youth under 21 years old complete a diagnostic assessment with a mental health professional to formulate goals and objectives identified by the child and their family. Treatment goals and interventions focusing on youth and family mental health needs are developed with a mental health practitioner in the family's home to:

  • Access Mental Health Behavioral Aide (MHBA) supports
  • Treat symptoms causing issues in the family environment
  • Improve family function through education, behavior management and skill building
  • Enhance daily living skills
  • Improve performance in school and recreational settings
  • Improve relationships

CTSS is reimbursable by Medical Assistance, some private insurance plans and contractual agreements with counties.

 

Prevention/Intervention Services
Our prevention and intervention services seek to:

  • Prevent out-of-home placements
  • Reduce family crisis
  • Provide after-care services to support reunification of youth returning to a family setting

Our caring, professional staff help children partner with their family and community by learning skills that create and strengthen new, healthy behaviors at home. All in-home services receive skilled clinical supervision by a licensed mental health professional.

 

Mental Health Behavioral Aide (MHBA)
An integrated part of the CTSS team, MHBA's provide practice, repetition, reintroduction and skill proficiency taught by the CTSS Mental Health Practitioner.

  • Child or family name seeking services
  • Child's date of birth and parent/guardian names
  • Insurance company and child's policy number

 

Targeted Case Management (TCM)
TCM assists children with Emotional Disturbances (ED) and their families with access to medical, social, education, vocational, financial and other needed mental health services. TCM services include:

  • Developing Individual Family Community Support Plan (IFCSP)
  • Ensuring coordination of services
  • Monitoring and evaluating delivery of services

 

Eligible children are up to 18 years old and have a mental health diagnosis that causes interference in multiple life areas such as school, home, peers or community. TCM recipient service activities include:

  • Assistance to obtain needed services
    • Coordinated Care
    • Relieve conditions leading to emotional disturbance and improve the personal well-being of the child - Improve family functioning - Enhance daily living skills
    • Improve functioning in education and recreation settings
    • Improve interpersonal and family relationships
    • Enhance vocational development
    • Assistance in obtaining transportation, housing, health services, and employment
  • Monitor discharge plan when leaving treatment facility
  • Health coverage assistance for covered medical services
  • Arrange travel, lodging, meals, etc. to advance access to services
  • Plan, assess and document all service coordination of a case
  • Receive supervision from a multi disciplinary clinical team
  • Develop goals, service plans, written service agreements and routine case supervisory activities
  • Participate in conferences, administrative reviews, and staffings

 

Families are not charged for TCM services. Charges are billed to Medical Assistance, IMCare and private insurances.
