Teens in Transition
Teens in Transition (TNT) is a specialized Group Residential program designed to promote independent living skills.Programming is oriented to assist young adult males in preparation for building life skills needed to live on their own. The TNT program is geared toward young adult males (ages 16 – age of majority) who would benefit from a transitional setting before living independently. With the assistance of professional staff members, each individual accepted into the program develops a treatment plan. This plan outlines the individual goals he hopes to achieve and a time frame for the achievement of those goals. The times frame for goal achievement is individualized to meet the needs and abilities of each person.
All participants will be required to seek and obtain employment as part of their treatment plan. They will have the opportunity to work with a job counselor at the Minnesota Office of Job Training. Participants will work with a counselor to develop a resume; complete applications for employment and learn interviewing skills.
In addition, participation in the Teens in Transition Program provides access to the complement of North Homes, Inc. programs including:
- Chemical dependency assessments, treatment and education
- Cultural programming
- Outpatient mental health services
- Psychological assessments
- Transportation
- Medical appointments can be scheduled at a local clinic or with a primary care provider
