35 Day Evaluation Program
The ITASKIN Center's 35-day Evaluation Program is designed for males and females 12 - 18 years of age. The program provides 24-hour behavioral observation and documentation by our professionally trained staff to assist our treatment team in identifying a child's and family's strengths and areas of interventions. The 35-day assessment consists of a number of components which are integrated into a final report including:
Behavioral: Staff members document resident's daily activities and interactions. These daily observations are a crucial component of the evaluation process and assist in the development of recommendations upon discharge.
Psychological: A licensed psychologist specializing in the developmental issue of children and adolescents will complete a psychological evaluation which many include a number of projective and objective measures, including cognitive assessment.
Psychiatric: A board-certified psychiatrist will conduct a psychiatric assessment to determine recommendations for medical treatment.
Academic: Educational programming is provided on-site by School District 318. Licensed teachers provide information which is incorporated into the final assessment report.
Family: A family interview is conducted to obtain historical information and to identify family strengths, values, and abilities to provide the necessary support for the child upon discharge.
Recreational: A recreational assessment will be completed to determine the individual's leisure time activities, sportsmanship and hobbies.
Physical: A physical examination will be scheduled at a local clinic or with a primary care provider. A dental and vision examination will also be scheduled as deemed appropriate.
Optional Assessments:
Cultural: A cultural assessment may be completed to address the individual's cultural and spiritual strengths.
Rule 25: A Rule 25 Chemical Dependency Assessment may be completed if concerns for chemical abuse are identified.
Psycho-sexual Evaluation: A psycho-sexual evaluation may be completed if the need is identified.
Neurologic Evaluation: If so warranted, ITASKIN Center Staff can assist in scheduling neurologic assessment to determine if other conditions may be impacting and individual's functioning.
* 35 Day Evlauations may require extension for completion of these options

* Community service/County work crew
* Educational Services
* Case management/Case planning
* Cultural programming
