Boy's Teens in Transition Program

(Age Group: 16-21st Birthday - Grand Rapids, MN)

Teens in Transition is geared toward young adult males who would benefit from a structured, home style transitional setting before living independently. With assistance of professional staff members, each individual accepted into the program will develop a treatment plan. This plan will outline individualized goals complemented by an incentive based level system. Program components include, but are no limited to:

  • Education
  • Daily skills groups
  • Individualized goal planning
  • Assistance with obtaining driver's permit/license, birth certificate, and social security card
  • Budgeting and money management
  • Structured daily recreation and leisure activities
  • Individual therapy
  • Cultural/Religious programming
  • Job search and coaching
  • Nutrition and meal planning
